Disabled Parking
Your healthcare provider’s office will need to provide you with the following:
- Application for Disabled Parking – Verify that the form is the most current available from DOL. It will need to be completed in its entirety.
- Prescription – Written on tamper-resistant prescription pad and manually signed by healthcare provider.
You will need to bring us the original copy of both forms.
